2. Name Circle/SSA Secretary, Address and telephone number.
3. Number of members in Circle/SSA
4. Main issues like non-payment of minimum wage, non-implementation of EPF/ESI, non-regularisation, victimisation if any etc.
5. Details of agitation conducted etc.
6. Other details, if any.

This may be treated as urgent.
